COMPLAINTS

To file a complaint with our facility, please submit a written complaint within 180 days of the suspected violation to info@plasticsmedspa.com. Please include as much detail as possible about the incident.

USE AND DISCLOSURE OF YOUR MEDICAL INFORMATION:
